Case Management services are available through Medicaid, Oregon Project Independence, and Aging and Disability Resource Connection (ADRC) / Senior Connections. There are many options for Case Management services and a Case Manager will work with you to determine the program that best meets your needs and situation.
Case Managers can help older adults and adults with disabilities who need personal care assistance to access the services and resources they need. Services include:
Options Counseling is a free service provided though the ADRC to all seniors and people with disabilities (18 and over), or their family members, to help them plan for their long-term care needs.
